New Patients

Getting started with services at GASLC is easy and our staff is here to help!

Your first step is to call our office (770) 977-9457. We will take basic information including your area(s) of concern, the services requested, schedule restrictions, and your insurance information. When you call the office please have your insurance information available. We will gladly contact your insurance company to determine your coverage and benefits for therapy services.

Prior to your first appointment we require our intake forms and any applicable assessment questionnaires to be completed prior to your appointment. These forms are all available online and are sent once a service is scheduled. Please contact our office if you have not received these forms prior to your appointment. If your child is in a Medicaid program and is receiving school or Babies Can’t Wait services we are required to obtain a current copy of the IEP/IFSP.
